The treatment of total, partial and unitary edentulism with osseointegrated implant supported prostheses is well documemted and is predictable with high long-term success rates. In cases where conventional treatment has limitations, the decision-making process to develop a plan looking for the best cost/benefit ratio presents the dilemma to maintain or replace teeth with a hopeless prognosis by osseointegrated implant supported prostheses in order to optimize esthetic, occlusal and phonetic functions, and also to avoid bone augmentation procedures. This paper aims to report two clinical cases in which the dilemma was solved by choosing to replace teeth with a dubious prognosis by osseointegrated implant supported prosthesis (total or partial) with early occlusal function.
