Background:
Adverse post-operative events and reoperation rates have been an important issue even if they are the markers of
surgical safety and therefore of great importance in guiding patients and surgeons in the choice of the more suitable operation.
Aim of the study:
In this study, we will analyze the most important complications in relation to each bariatric surgery regarding
the rate, severity and the comparison between three different procedures (SG, MGB and Roux-en-Y).
Methods:
Combined retrospective and prospective single center observational study based on the data extracted from the Basra
Society of Bariatric Surgery and Metabolic Disorders database, which covers almost all the bariatric operations performed in
Basra. We analyzed the 30 days post-operative complications occurring, in the period from 2012 to 2016, after Roux-en-Y Gastric
Bypass (RYGB), Sleeve Gastrectomy (SG) and Mini Gastric Bypass/One Anastomosis Gastric Bypass (MGB/OAGB).
Results:
In the period between 2012-2016, a total of 1100 operations were performed, of which 132 RYGB (12%), 857 (78%) SG
and 111 MGB/OAGB (revision procedures were excluded). The age of the patients are ranging from 17-63 years with mean age of
(40±2) and BMI of 34-52 kg/m2. The complication rate was 9.09%, 4.08% and 7.2% respectively. The most frequent complications
were hemoperitoneum (0.63%), perforation or visceral injuries 0.27%, leak rate less than 0.3% and portomesenteric thrombosis
0.09% that were lower in SG when compared with RYGB and MGB/OAGB.
Conclusion:
This study supports a safe profile of obesity surgery in Basra, along with positive bariatric outcomes. The rate of 30
days post-operative complications is progressively lower after SG and OAGB, OAGB records statistically less low complications
compared to RYGB. The mortality mainly related to thromboembolic phenomena and pulmonary embolism.
