Difficult vascular access in low profile EVAR: how it modified our practice and outcomes

Abstract:

The importance of the anatomy of the aortoiliac segment in EVAR and TEVAR .The suitability for and durability of EVAR and TEVAR is determined by the anatomy of the aortoiliac system.Complex aortoiliac system with small, narrowed, severely calcified or tortuous iliac arteries make EVAR and TEVAR more complex. Iliac angioplasty, iliac external conduits, iliac endoconduits with covered stents may be needed prior interventions for better accommodation of the delivery system.Technical and technological advances have increased the patient eligibility for EVAR (endovascular aneurysm repair) and safety of EVAR procedures. The last generations of low profile represent advancement in endovascular technology and widen the availability of EVAR and to include many patients with poor vascular access. Also more flexible covered stents grafts and stiffer wires have allowed us to treat more complex cases without the necessity of iliac conduits. Low profile devices increase the possibility of percutaneus (PEVAR) EVAR with local anesthesia.

Angel J Menendez Aponte is a Vascular Surgery Specialist in San Pablo Medical Center, United States. He was Graduated with honors in 1980. He is having more than 39 years of diverse experiences, especially in vascular surgery& thoracic surgery.
